Does the amex car insurance with a plat card cover hire cars in Italy (I understand there are some countries that are restricted but couldn’t find the details)
I don’t see why not. Per the terms the only restrictions are “locations where a government agency has advised against travelling”.
Amex Platinum Insurance Terms and Conditions
There are no countries where it is excluded (except, as noted above, where the Government advises you don’t travel). You
The only issue is how casual rental firms are over you declining insurance. In the UK there is a legal cap on your liability anyway, but this isn’t the case globally. You need to be prepared to give a card swipe for the full value of the car if necessary – after all, the rental company can’t prove you have cover.
I am wondering the same thing as have a car hire booked in Italy with Hertz. It seems that by law the car hire firm has to provide CDW, in which case the Amex Platinum insurance cannot be used, as Herts are not able to remove the CDW element.
Amex Platinum US specifically excludes Italy from its car hire insurance for this reason. However, Amex UK does not but for the reason above it will never be covered.

Picking this up as I have just come across it but have rented several times in Italy after specifically speaking to the Amex’s insurance provider.
Amex Platinum UK will cover the optional insurance that are offered by the Car Hire company, so the fact the CDW is included in the basic insurance which is part of the price paid for the rental, does not affect this (Amex will not pay that portion already paid by the basic insurance).
Amex will basically only cover the supplemental liability which is sold as a separate cover by the car hire companies (e.g. if the car is written off or stolen the Supplemental liability is usually €2500-3000 usually)
